मी, श्री./श्रीमती , वय वर्षे, व्यवसाय , राहणार , याद्वारे शपथेवर प्रतिज्ञापूर्वक लिहून देतो / देते की,
- मी या जातीचा / जातीची असून, ती या प्रवर्गात मोडते.
- माझे जात प्रमाणपत्र क्रमांक , दिनांक , यांनी निर्गमित केलेले आहे.
- सदर जात प्रमाणपत्र पडताळणीसाठी मी यांच्याकडे अर्ज सादर करीत आहे.
- सदर पडताळणी या प्रयोजनासाठी आवश्यक आहे.
- माझ्या अर्जासोबत सादर केलेली सर्व कागदपत्रे व त्यातील नोंदी खऱ्या व बरोबर असून, त्यात कोणताही फेरफार अथवा खाडाखोड केलेली नाही.
- माझ्या जातीबाबत यापूर्वी कोणत्याही पडताळणी समितीने माझा दावा अवैध ठरविलेला नाही, तसेच माझा प्रस्ताव कोठेही प्रलंबित नाही.
- सदर प्रतिज्ञापत्रातील मजकूर खोटा आढळून आल्यास, महाराष्ट्र अनुसूचित जाती, अनुसूचित जमाती, विमुक्त जाती, भटक्या जमाती, इतर मागासवर्ग व विशेष मागास प्रवर्ग (जातीचे प्रमाणपत्र देण्याचे व त्याच्या पडताळणीचे विनियमन) अधिनियम, २००० मधील तरतुदींनुसार होणाऱ्या कारवाईस मी पात्र राहीन.
वरील मजकूर मी वाचला असून तो माझ्या माहितीप्रमाणे व समजुतीप्रमाणे खरा आहे.

How to use it
- Fill in the applicant's details and the caste exactly as it is spelled on the caste certificate — spelling differences are the single most common reason a scrutiny file is returned.

- Print it on stamp paper of the value your district requires, sign it before a notary, and file it with the scrutiny committee.

What is Form-17?
The affidavit that goes with an application to a Caste Scrutiny Committee for a caste validity certificate. The applicant swears that the caste claimed is genuine and that the supporting documents are true.
When do I need caste validity?
For admission to a reserved seat in professional courses, for appointment against a reserved post, and for contesting a reserved seat in local body elections. The caste certificate alone is not enough — the committee must validate it.
Plain paper or stamp paper?
An affidavit is sworn, so it goes on stamp paper and is notarised. The stamp value differs by district; ask the committee office or your notary.
What documents go with it?
The caste certificate, the validity certificates of blood relatives if any exist, school records showing the caste entry — for the applicant, the father and ideally the grandfather — and the genealogy. The family tree maker produces that last one.
