How to use it
- Choose whether it is a birth or a death, and fill in the details of the person and the event.
- Write the real reason the registration was missed — poverty, a home birth, illiteracy, distance from the office. A vague reason gets the file returned.
- Print the application. It goes to the Tahsildar or the Sub-Divisional Magistrate, depending on how late the registration is.
- Attach the supporting documents listed at the bottom of the page.
Questions about this tool
How late is "late"?
Under the Registration of Births and Deaths Act, within 21 days is normal registration. Up to 30 days the registrar can accept it with a late fee. Between 30 days and a year it needs written permission from the prescribed authority. After a year it needs an order from a first-class magistrate or an executive magistrate — in Maharashtra, usually the SDM or Tahsildar.
What evidence will they want?
Hospital or ANM records if there are any; the school leaving certificate showing the date of birth; the gram panchayat register; an affidavit from the parents; and often statements from two independent witnesses from the village.
How long does it take?
Weeks to a few months, depending on the office and whether an enquiry is ordered. Filing a complete set the first time is the only thing that reliably speeds it up.
Can I use this for a name correction?
No. A correction to an existing entry is a different application under Section 15 — this one is for a registration that was never made at all.
