मी श्री./श्रीमती/कु. , वय वर्षे, राहणार , ता. , जि. येथील वरील ठिकाणी राहत आहे. मी अधिवास / राष्ट्रीयत्व प्रमाणपत्र मिळण्यासाठी अर्ज केला असून, कोणत्याही अन्य जिल्ह्यात किंवा तालुक्यात यापूर्वी अधिवास प्रमाणपत्र व राष्ट्रीयत्व प्रमाणपत्रासाठी अर्ज केलेला नाही, किंवा यापूर्वी कोणत्याही अन्य जिल्ह्यातून/तालुक्यातून असे प्रमाणपत्र मिळविलेले नाही.
तरी मला अधिवास प्रमाणपत्र / राष्ट्रीयत्व प्रमाणपत्र मिळावे, त्यासाठी हे स्वघोषणापत्र लिहून देत आहे.
वरील माहिती खरी असून खोटी आढळल्यास भारतीय न्याय संहिता २०२३ मधील कलम २३६, २३७ अन्वये शिक्षेस पात्र राहीन याची मला जाणीव आहे.

What are these two pages for?
Page 1 declares that you have not applied for, or already hold, a domicile / nationality certificate from any other district or taluka — the actual अधिवास व राष्ट्रीयत्व प्रमाणपत्र self-declaration. Page 2 is a separate भूमिहीन (landless) self-declaration some Tahsil offices ask for alongside it.
Do I need the landless declaration too?
It depends on the office. Some ask for both as one self-declared packet; others need only page 1. Ask the office issuing the certificate, or whoever is filing on your behalf.
What is a domicile certificate for?
It proves continuous residence in a state or district — needed for education seats, jobs and schemes reserved for local residents. A nationality certificate is often applied for alongside it using the same declaration.
What proof should I attach?
Usually a school leaving certificate showing the address, a ration card, Aadhaar, and — for a minor — a parent's own domicile certificate. The office's own list is final.
Why does selecting Surgana fill in some of this already?
