मी. रा. ता. जि. येथील रहिवाशी असून सात/आठ अ प्रमाणे माझ्या नावे मौजे मध्ये एकूण क्षेत्र हे.आर एवढे (स.नं./गट नं. ) असून सन हंगाम या वर्षाकरिता सदरील शेत जमिनीमध्ये खालील पिक घेतली जात आहेत. त्याकरिता खालील प्रमाणे पिक पेराचे स्वयंघोषणा पत्र देत आहे.

टीप : विम्याच्या संदर्भात केंद्रचालक कोणत्याही जबाबदारीस पात्र राहणार नाही. फक्त पीक विमा भरल्याची बिनचूक पावती देणे ही केंद्राचालक ची जबाबदारी आहे.
वर दिलेली सदर माहिती ही खरी असून चुकीची आढळल्यास कायदेशीर कार्यवाहीस सर्वस्वी मी स्वतः जबाबदार राहिन.

सदर सहखातेदारांपैकी कोणीही भविष्यात ह्याबाबत तक्रार किंवा हरकत उपस्थित केल्यास त्यास सर्वस्वी जबाबदार मी असेन. उपरोक्त सर्व नमूद केलेली माहिती खरी असून त्याचे सत्यतेबाबत मी खात्री केली असून यामध्ये काहीही चुकीची / खोटी माहिती आढळून आल्यास मी भारतीय दंड संहिता १८६० चे कलम १९३(२), १९९, २०० च्या तरतुदी नुसार कायदेशीर दंडास पात्र असेन, याची मला संपूर्ण जाणीव आहे.

How to use it
- Fill in your details, the survey/gat number, and the season and year — both update the title and the declaration text as you change them.
- List what you have actually sown, one row per crop — the sowing date and area (split into हेक्टर and आर automatically) fill in as you type.
- If this land is a सामाईक (jointly-held) account, tick that box — a second page, the सामाईक सहमतीपत्र, appears for the co-owners to sign.
- Print both pages, sign them, and attach the 7/12 extract when you file the crop insurance application.

What is the pik pera declaration?
पीकपेरा बाबत स्वयंघोषणा — the farmer's own statement of which crop is sown on which plot and over how much area, in the format set by the कृषी, पशुसंवर्धन, दुग्धव्यवसाय विकास व मत्स्यव्यवसाय विभाग's शासन निर्णय क्र. प्रपीवीयो./२०२०/प्र.क्र.४०/११ऐ, दि. २८ जून २०२०. PMFBY requires it with the insurance proposal, because the premium and any claim are calculated from it.
Why does the area have to match the 7/12?
The insured area cannot exceed the area you hold. If the declaration says more than the 7/12 extract shows, the proposal is rejected at scrutiny, or the claim is cut at settlement.
What if I have sown more than one crop?
List each crop on its own row with its own sowing date and area. The rows add up to the total sown area, which should not exceed the plot.
What is the सामाईक सहमतीपत्र for?
When the 7/12 is a सामाईक (joint) account with more than one खातेदार, the insurer needs the other co-owners' written consent to insure your share of it — this second page is exactly that consent form, with a signature line for each co-owner. Skip it (leave the box unticked) for a plot that is not jointly held.
What happens if the declaration is wrong?
A false declaration voids the insurance and the premium is not returned. Field verification does happen. Write what is actually in the ground.
